Chadwick Boseman was buried near his South Carolina hometown six days after he died at his home in Los Angeles, according to a death certificate released on Monday.
Boseman died at his home near Griffith Park in Los Angeles on Aug. 28, the record said.
The Black Panther star was laid to rest Sept. 3 at Welfare Baptist Church Cemetery in Belton, South Carolina, according to The Associated Press.
The Los Angeles County Certificate showed Boseman’s hometown of Anderson, roughly 11 miles away from his burial site.
His immediate cause of death was listed as multiple organ failure, with the underlying cause of colon cancer, which his family said previously that he had been diagnosed with four years earlier.
Boseman had surgery to remove the colon cancer in 2016 after his diagnosis, and in March of this year had laparoscopic surgery to remove cancer that had metastasized, the record showed.
The document lists Boseman’s profession as ‘artist’ in the ‘entertainment’ industry.
Chadwick is mostly recently remembered for his inspiring performance as King T’Challa in the 2018 film Black Panther, but he first appeared as the powerful character in the 2016 MCU flick Captain America: Civil War.
Directed by Ryan Coogler, the film earned a host of nominations and awards, and garnered more than $1billion at the global box office.

He largely battled cancer in secrecy, with very few people outside of his family aware of his diagnosis.
Stars of stage and screen gathered in Malibu for a small memorial service nine days after his death, including Michael B. Jordan, Lupita Nyong’o and Winston Duke.
Boseman’s death was announced on his personal social media accounts saying in part ‘He died in his home, with his wife and family by his side.’
Chadwick is survived by wife Taylor Simone Ledward; the couple reportedly married earlier this year after dating for five years.
